Senior decisions are almost never made with enough information. The skill is not in achieving certainty but in being explicit about which bet you are placing and what would change your mind.

A decision record does not need to be elaborate: the question, the options considered, the assumption the choice rests on, and the signal that would justify revisiting it.

Two years later that paragraph is worth more than any slide, because it lets your successor distinguish between a decision that was wrong and a decision whose assumptions expired.
